A care pathway map is a visual representation of a care pathway, typically in the form of a one-page flowchart diagram, which clearly describes the different elements of care, and services/teams involved, to health professionals and/or service users .

Care pathways seek to promote organised and efficient client care that is built upon evidence-based practice. Evidence shows that the implementation of care pathways significantly reduces the variability in clinical practice and improves outcomes.

  • How data can be used to understand pathways and identify improvement opportunities

This will give participants the core knowledge required to undertake work around pathway redesign, and pathway implementation.
