This webinar is for a wider national audience of; PCN teams, commissioners, ICS workforce and transformation leads and other’s working in either place-based, or integrated care system roles, as well as link workers, to support the development of your social prescribing workforce, making the most of your social prescribing ARRS roles.

What can social prescribing ARRS roles bring to Population Health Management?

This webinar will be presenting a series of case studies, initiatives and platforms, describing how key elements of the Social Prescribing Link Worker (SPLW) workforce role is developing in different parts of the country and working effectively using population health management approaches to:

  • Recruit and develop SPLWs roles, including development of skills and qualifications for career progression and staff retention.
  • Using data led approaches targeting SPLW roles at a PCN level to reduce health inequalities
  • Supporting people to unpick complex issues affecting their wellbeing and improve outcomes.
  • Demonstrating the impact that social prescribing can have utilising and responding to population health data and the impact of this approach in terms of additional recruitment.

There will be opportunity for social prescribing teams to gain skills and insights in using population health approaches, demonstrating social prescribing as a pivotal part of wider team population health management approaches.
