The GM Dementia Wellbeing Plan has already been successfully piloted and Health Innovation Manchester together with Dementia United, part of GMICP, are now planning the digitised version to be piloted in two localities, prior to rolling out across the whole of GM. This will provide the structure for improved communication and integration of care across all.

As well as presentations & discussion, we want to develop a virtual Clinical Reference Group to advise our Project Delivery Group which is led by Dr Saif Ahmed.

This event is relevant for all who have contact with people living with dementia and their carers including, but not limited to: GPs, Consultants, Primary Care Health Workers, Memory Assessment Service staff, Dementia Advisors in the VCSE, Adult Social Care staff, Community Mental Health Team staff, Local Care Organisation staff, e.g. social workers, nurses, medical staff, AHP, general hospital staff, Care Home staff, Day Centre Services staff, Hospice staff, Ambulance workers and people working in sensory impairment retail outlets e.g. Spec Savers, Vision Express etc

This event is part of a broader consultation process which includes people living with dementia and their carers. Further info from sarah.kirkland@nhs.net (planning) or saifahmed@nhs.net (clinical).
