The Cheshire West Integrated Care Partnership (ICP) has developed its transformation priorities for 2019 / 20 and has identified 4 Priority Programmes and 6 Enabling Programmes. The Priority Programmes are:-

• We will develop 9 Care Communities across Cheshire West to deliver our Model of Care

• We will develop Intermediate Care services which take away services that don't need to be delivered in an acute setting into community

• We will support people living with Long Term Care needs to receive the right services in their community

• We will support people to enjoy Healthy Lives in their communities

 

The ‘Thinking and Acting Differently for Transformation’ Programme will provide a collaborative approach to:-

• Align common purpose across the Cheshire West ICP

• Redesign, improve and transform health and care across Cheshire West

• Create opportunities for continuous learning and innovation

• Share responsibility and opportunity across the whole system

• Create ‘Explorers’ for transformation from across the system

• Create action learning forums

• Develop new priorities and working models for change

• Use our system OD and Improvement Leads to support the programme

• Shape a new sustainable culture for transformation

 

Each day long session will provide the place, space and time to:-

• Work on your ideas and challenges which will support the vision of the ICP

• Bring together the best of our collective knowledge and resources

• Bring together the full range of community assets to improve population health

• Understand what will make a difference in the short, medium and long term

• Work on what could be different and not more of the same

• Make community-based care the central focus of planning and provision at all levels
